Arizona State Archives staff will make every reasonable effort to respond to requests for research information as quickly and thoroughly as possible. Staff will not perform open-ended research for patrons that would require them to interpret the records. This includes legal, genealogical, land title and ownership or in-depth historical research. Each off-site public patron request will be limited to a maximum of two names and subjects and one record per name and subject. In-state off-site public patrons will be allowed up to one-half hour of free research twice every four months. Out-of-state off-site patrons will be allowed up to one-half hour of free research once every four months. Additional requests within the four month period will result in a research free of $15 per request for both in-state and out-of-state off-site public patrons.

NOTE: Requests for information from newspapers (obituaries, article look-ups etc) must use a separate form.

Record Information

First name of the person whose record is sought.
Middle name of the person whose record is sought.
Last name of the person whose record is sought.
Date of requested record: Be as specific as possible. Limit estimates to ranges of ten years or less. Do not enter today's date.
County, City or Town: Usually this is the county, city or town of residence for the person whose record is being requested. For death certificates, use the county, city or town in which the person died. For land records use the county, city or town in which the land was located. If you do not know the location, please type unknown.)
Other pertinent or helpful information: For example, names of children, parents or spouse, occupation, length of residence in Arizona or any other identifying information.
