LSTA Final Report


The 2009 LSTA grant final report is due Sept. 3, 2010. All funds must be expended by Aug. 2, 2010; if you have any concerns that are keeping you from completing your project, please let me know immediately.

The final report for 2009 LSTA grants is available at http://www.lib.az.us/alts/Login.aspx.

  1. You will login with your e-mail address and user password. The default password is password. Once you login, select “Users” from the left side menu and reset your password.
  2. Then, select “Grants” from the left-side menu.
  3. You can find your grant by selecting “Library Name” and scrolling through the list of libraries. Your grant will be listed under the library system name, not the branch name. If you are associated with a university, it will be listed under the university name.
  4. Unless you select other criteria, all recent grants to your library will be displayed. Click on the grant project you want to report on.
  5. You will be asked for a GRANT password. This is the password that you selected when you set up the grant in March 2008. If you don’t know your password (and I wouldn’t after 16 months) send me an e-mail and I’ll let look it up for you.
  6. Select the words “Final Report,” which will be toward the top of your screen. You’ll complete the Cover Sheet, Worksheet, Narrative and Fiscal Report.
  7. The Narrative requires you to provide “short essay” responses. You may find it easier to write these in a text editor and paste them into the space. Please keep the answers short. Don’t use tables or bullets as they won’t transfer to the federal report that I’ll move your data to.
  8. Be sure to edit the budget to indicate your actual expenditures, and update cost share.
