LSTA Final Report

The final report for 2008 LSTA grants is available at http://www.azlibrary.gov/alts/Login.aspx.

1. You will login with your e-mail address and password. If you have not used this system before (most likely to apply for a 2009 grant), your password is the word password. Once you login, select “Users” from the left side menu and reset your password.
2. Then, select “Grants” from the left-side menu.
3. You can find your grant by selecting “Library Name” and scrolling through the list of libraries. Your grant will be listed under the library system name, not the branch name. If you are associated with a university, it will be listed under the university name.
4. Unless you select other criteria, all recent grants to your library will be displayed. Click on the grant project you want to report on.
5. You will be asked for a GRANT password. This is the password that you selected when you set up the grant in March 2007. If you don’t know your password, send an e-mail to lstone@lib.az.us. Select the words “Final Report,” which will be toward the top of your screen. You’ll complete the Cover Sheet, Worksheet, Narrative and Fiscal Report.
6. The Narrative requires you to provide “short essay” responses. You may find it easier to write these in a text editor and paste them into the space; I’m attaching a copy of the questions to this e-mail. Please keep the answers short. Don’t use tables or bullets.
7. Your fiscal report will only have the grant total. You’ll need to enter categories and amounts indicating what you spent.

The evaluation is due Sept. 4, 2009. All funds must be expended by Aug. 3, 2009. If you anticipate not meeting these deadlines, please let me know immediately. If you run into problems or need assistance with the online report, contact lstone@lib.az.us.
