LSTA Final Report Forms
A final report, including project data, narrative and budget summary, must be completed for each LSTA project. The report has two parts:
Part 1 - Cover Sheet and Budget Form: This is a “fill-in-the-blanks” form. You’ll see a number of different check lists. Select the responses that best fit your project. Please mail this part of the form to Laura Stone by Sept. 4. Be sure to include any publications, publicity materials, or photographs.
Part 2 – Narrative: Please provide a short answer to each of the questions. A three- to five-sentence paragraph should be sufficient to answer each of the questions. Please e-mail this part of the report to lstone@lib.az.us.
