If you are referencing the State Filed Form 470 Number 163890000546479 for your Form 471 and are using the Telecom and Internet contracts dated (1/13/2006- 1/12/2009) of the following vendors

- WJ Vod Corporation (No SPIN Number)

- AT&T SPIN Number 143026737
- COX SPIN Number 143014467
- Mountain Telecom SPIN Number 143005449
Please note the 471 procedures
- This contract expires on 12 Jan 2009 (which is the middle of the funding year) so any entity using this 470 will have to file 2 form 471s one for the period 7/1/2008-1/12/2009 and a second form 471 for the period 1/13/2009-6/30/2009
- For the second Form 471 you have to use the Replacement SPIN number 143999999 (This is a generic number and this is how the computer tracks that there is going to be a SPIN change later. Even if the contracts are being extended with the same vendor)
- Both forms have to be filed before Feb 7th 2008.
- Then once the new contracts (extensions) are in place (in Jan, 2009) you will have to send USAC a SPIN change letter to reflect the correct SPIN number of the new contracts for the second 471 1/13/2009-6/30/2009.
- You cannot file just one form 471 as you would have done in previous years.
